Documents to dive in Spain

IMPORTANT
IF YOU ARE PLANNING TO DIVE IN SPAIN READ THIS DOCUMENT CAREFULLY

THERE ARE 3 NECESSARY DOCUMENTS YOU MUST POSSESS TO BE ALLOWED TO DIVE IN SPAIN

1- DIVING CERTIFICATION

The diver must be certified by Padi Scuba diver or equivalent

ATTENTION: In the Medes Islands: Padi Open Water Diver or equivalent

2- DIVING INSURANCE

You must be covered in the event of an accident by practicing scuba diving. However, most basic travel insurance or personal healthcare don’t include scuba diving. Please make sure you possess such an insurance and you have a proof (card, screenshot, etc...)

Otherwise, we can make one on site. We offer one day insurance for 9€, one week insurance for 16€, a month insurance or a year insurance. In order to do this, we will need your surname, first name, date of birth, ID card number, email address and telephone number.

3- MEDICAL QUESTIONNAIRE FULLY COMPLETED

In this link you can fill out the medical questionnaire: https://www.unisub.es/en/Diver-Medical-questionnaire. Below, we provide an explanation on how to complete this document properly. Please, read it carefully:

In the medical questionnaire there are yes/no questions. If the form says SUITABLE, you only need to fill out the rest. However, if there is a NON SUITABLE, you need to download the document from the folder below: Medical Questionnaire. The third page from the Medical Questionnaire needs to be filled out by a medical professional.

ATTENTION: It is mandatory to complete this form even in case of having a medical certificate.
If you have a separate medical certificate (not signed on the 3rd page) it is not valid in Spain.
